Residency / Fellowship Application Process


RESIDENCY / FELLOWSHIP APPLICATION PROCESS

Enclose the following documents in a LONG BROWN ENVELOPE with the applicant’s full name (LASTNAME, FIRST NAME, M.I. ). Indicate the Residency / Fellowship Program you are applying and submit  to the office of DEPARTMENT OF MEDICAL EDUCATION AND  RESEARCH (DMER) located at  6th Floor Clinical Division, UST Hospital.

REQUIREMENTS:

1. Letter of Intent addressed to:

MARCELLUS FRANCIS L. RAMIREZ, MD
Chair, Department of Medical Education and Research (DMER)
University of Santo Tomas Hospital 

2. Application Form and Application Fee at DMER Office.

3. Recent ID pictures (2 pcs. passport size, colored on white background)

4. Letter of Recommendation (LOR) -  TWO LETTERS preferably from the Dean, Clerkship Director, Internship Director, Residency / Fellowship Training Officer and / or  Department Chairman/Chief of Section. (All LOR must be addressed to the DMER Chair)

 5. Certified True Copy OR Notarized Copy of the following:

5.1 Class Ranking
5.2 Transcript of Records
5.3 Medical School Diploma
5.4 Certificate of Internship
5.5 PRC Certificate
5.6 Updated PRC License
5.7 Itemized PRC Board Rating
5.8 Residency Certificates (for Fellowship applications only)
5.9 Diplomate Certificate (for Fellowship applications only)
5.10 Birth Certificate (photocopy)
