Enclose the following documents in a LONG BROWN ENVELOPE with the applicants fullname (LN, FN, MI) written at the left uppermost corner of the envelope. Indicate the Residency/Fellowship Program the he/she is applying in below his/her name and submit to the Deparment of Medical Education and Research

Fully accomplished application form of UST Hospital GME with 3 passport size, recent ID pictures on white background
- Two Letters of recommendation preferably from the Clerkship Director, Internship Director, Residency/ Fellowship Training Officer and/or Department Chairman/ Section Chief

- Birth or Baptismal Certificate
- Class Ranking
- Itemized PRC Board Rating
- Transcript Of Records
- Certificate of Internship
- PRC Certificate
- Medical School Diploma
- Residency and Diplomate Certificates
for applicants in Fellowship Programs
- Pay to the Cashier the amount of PHP 300 for the application processing and entrance examination fee.
THIS IS THE ONLY AMOUNT TO BE PAID TO USTH AT THIS STAGE OF THE SCREENING PROCESS.
- Take the examination and interview on the scheduled date.

NOTE: Foreign applicants must submit copies of the following duly authenticated by the Philippine Embassy from their country of origin. Certification or Letter of assignments as general practitioner in country origin.
- Transcript of Records
- Diploma or Certificate of Graduation/Completion
- And other supporting documents required by the Philippines
Embassy from their country of origin
- OTHERS: TOEFL results if available is an advantage but
not yet required
- True copy of applicant's passport showing admission status
and update stay plus passport pages where name, photo,
birth date and birth place appear.
Foreign applicants cannot displace natural born Filipino citizens from any training slot, Foreigners can be accomodated where there is vacancy.
